By completing this commitment form for my child for Unwritten 2026, I agree to the following terms:*

I confirm that my child is available for and will attend their usual lessons in dress rehearsal week (27^th, 30^th June & 2^nd July).*

I understand that regular attendance to lessons is essential for my child to learn their routines.*

I give permission for my child to be photographed and videoed as part of Unwritten 2026 and I understand that these photographs and videos will be used by Infinity Dance Academy for advertising purposes and/or for use on our website/social media sites (no full names will be given).*

I agree and understand that the first half of the payment for costumes made with this form is non-refundable should my child no longer be able to take part in Unwritten 2026*

I confirm that I have selected the correct costumes for my child on this form (please refer to class times on invoice if unsure).*

I agree to purchase the remaining balance by completing the costume form for my child’s costumes by 28^th March 2026.*

I confirm that I have read, understood and agree to the show info (taking part, costume info, tech run info, show weekend, dress rehearsal info, tickets info).*
